Patient Information

These few pages contain important information that you as a patient of Woodlands should be aware of, they detail some aspects of what you can expect from us as a practice and also what we can reasonably expect from you as our patient.

Just follow the links above.

As our main aim is to provide expert and friendly personal health care for you and your family we would be very interested in hearing your suggestions and ideas of how our system could be improved.

A Suggestion box is available for all your comments in the reception area, alternatively you can contact us by email here - woodlands.fmc@nhs.net

Here are some simple ways for you to help us to help you

  1. Let us know of any changes in names, addresses and phone numbers so that our records can be kept up to date.
  2. Try not to be late for your appointment.
  3. Inform us as soon as possible if you are unable to keep your appointment.
  4. Remember to allow 48 hours for a repeat prescription.
  5. Do not ask for a home visit if it is not essential.
  6. Do not expect the doctor to see two members of your family when you have only booked one appointment
  7. Keep your telephone calls short and make non-urgent calls in the afternoon.

    Thank You
